
Care Manager Job Description
Dignity Care, a premier locally owned homecare agency, is seeking a Care Manager to join our dedicated and skilled home care team. Our Care Managers serve as the primary liaison between our clients, their families, caregivers, and other healthcare professionals. This position includes some caregiving hours.
RESPONSIBILITIES
- Perform client assessments/intakes
- Train and supervise care giving teams
- Coordinate monthly team meetings
- Communicate regularly with clients and their families
- Develop and assess client Care Plans
- Uphold our compassionate high standard of care
- Maintain professionalism at all times
- Work caregiving shifts when needed
QUALIFICATIONS/DESIRED TRAITS
- Minimum of 2 years care giving experience
- Care manager, social worker, CNA, or equivalent
- Excellent interpersonal and written communication skills
- Extremely responsible and compassionate
- Self-motivated, but also a team player
- Good conflict resolution skills
- Skilled in finding creative solutions to address a diversity of homecare challenges
- Comfortable supervising care giving teams
- Maintain attention to detail without loosing sight of the big picture / overall priorities
SALARY
Care Giving: Will start at $11/hour with potential to increase to $13/hour after a trial period.
Care Management: Will start at $14/hour with potential to increase to $15.50/hr after a trial period.
Case Management: $25/hour
We also have plans to initiate quarterly profit sharing measures starting in Dec of 2006.
