Geriatric Care Management
Each client of Dignity Care is provided with a Client Care Counselor. Dignity Care takes a team approach to caring for elders. The Client Care Counselor is the primary contact for the elder and the family, encouraging the caregivers to follow their wishes. A team of caregivers is assembled and acclimated to the needs of the client and family. The Client Care Counselor makes sure that the client, family, and caregivers are all on the same schedule.

  • Doctor appointments and follow through with instructions
  • Collaborating with other agencies and facilities
  • Complex case coordination
  • Collaboration with RN for effective medication
